Cockcroft-Gault Equation:
Male: [(140 - age) × weight (kg)] ÷ [0.815 × SCr (micromol/L)]
Female: {[(140 - age) × weight (kg)] ÷ [0.815 × SCr (micromol/L)]} × 0.815
Ideal Body Weight:
Male: 50kg + 0.9kg/cm (for each cm over 152cm)
Female: 45.5kg + 0.9kg/cm (for each cm over 152cm)
Refer to Ideal Body Weight Calculator.
Creatinine clearance (CrCl) is the volume of blood plasma cleared of creatinine per unit time. It can be estimated using serum creatinine levels. The Cockcroft-Gault Equation uses a patient’s weight (kg) and gender to predict CrCl (mg/dL). The resulting CrCl is multiplied by 0.85 if the patient is a female to correct for the lower CrCl in females. The formula is dependent on age as its main predictor for CrCl.[2]
Please be noted that under the gender option, only "male" or "female" are considered as the natural biological gender. Although calculation is still theoretically feasible, the minimum height requirement for the accuracy of this formula is 152cm for both genders. These are also due to the requirements of the formula according to AMH.
The IBW could be used while calculating the dose for an overweight or obese patient, where the actual body weight would be significantly higher. The discordance could lead to potential misdosing.
